Registration

Registration is CLOSED for the 2024-2025 school year.

What You Need to Have:

NEW Family Registration(include the following):

New Families to the program are asked to come into the office to register as there are documents that are required along with questions to be answered.  If at all possible, students should come with parents so the office staff can meet them.
 

RETURNING Family Registration(include the following):
 

HOMESCHOOL Family Registration (include the following): Make sure that you register with RE for important sacramental information updates.
 

OR

• Need to Register with the Parish office.

• Registration Form 

• NEW Student  Form (1 for each student)

• Copy of Baptismal certificate for each student (mandatory)

• First Communion Certificate (if applicable)

• Letter from prior parish confirming attendance and completion of grade(if applicable)

• Required Participation in Parish Activites Waiver (for each student)

• Photo, Press, Audio & Electronic Release Form (for each student)

• Student Code of Conduct ( for each student)

• Registration Form

• New Student Form (younger siblings)

• Any changes to contact information, ie. Home phone, cell phone, email, and/ or street address.

• Required Participation in Parish Activities Waiver (for each student)

• Photo, Press, Audio & Electronic Release Form (for each student)

• Student Code of Conduct (for each student)

• New Homeschool Family form

• New Homeschool Student form 

• Baptismal Certificate for each student (mandatory)

• First Communion Certificate (if applicable)

• Returning Homeschool Family form
